You MUST include a copy of your valid Texas driver’s license.

You MUST include proof of personal auto insurance coverage or proof of financial responsibility.

This form MUST be signed in front of a notary.

TO BE ELIGIBLE FOR A DRIVERS SAFETY COURSE

  • You MUST have a valid TEXAS DRIVERS LICENSE
  • You MUST provide evidence of financial responsibility Code of Criminal Procedure Art. 45.0511(b)(6).
  • You CANNOT hold a CDL or CDL PERMIT Code of Criminal Procedure Art. 45.0511(s).
  • You CANNOT have been going more the 25mph over the speed limit or at a speed of 95 mph or more. Code of Criminal Procedure at. 45.0511(b)(5).
  • You CANNOT have been going more than 95 mph.
  • You CANNOT have committed the offense in a construction zone with workers presented. Code of Criminal Procedure Art. 45.0511(p)(3);
  • You CANNOT have taken a Drivers Safety Course in the past 12 Months for a ticket dismissal. Code of Criminal Procedure Art. 45.0511(c).
  • You CANNOT have been Passing a school bus while loading/unloading children, Code of Criminal Procedure Art. 45.0511(p)(1).0
